Office Policies
Payment
Payment is expected at the time of service. This includes co-pays and/or deductlbles if we are filing insurance for you. We accept cash, checks, and major credit cards. We reserve the right to reschedule your appointment and/or charge a $10.00 fee if you are unable to meet your co-pay obligation.
Referrals
If your insurance plan requires a referral from your primary care physician, you are responsible for obtaining a referral prior to your appointment. If you do not have a referral, your appointment will need to be rescheduled or you can be seen as a self-pay patient.
Medical Records Charge
There is a $35.00 fee for copies of medical records.
Disability and FMLA Forms
We will be happy to complete any disability or FMLA forms you may require. Please allow 7-10 business days for the completion of these forms. FMLA forms will be processed after your scheduled procedure. There will be a $25.00 charge for these forms to be completed.
Returned Check Policy
There is a $50.00 fee for returned checks.
No-Show Appointments
Lipman Mandell Retina Center reserves the right to bill you for a missed or "no show" appointment without appropriate notice of cancellation. After two consecutive "no shows", there will be a $50.00 fee applied to your account.
